You can save time at your first appointment by completing the Patient Registration Forms before your visit.
Appointments and Cancellations
Patients are seen by appointment only. Please call 24 hours in advance to cancel appointments you know you will miss. We charge $50.00 for a regular missed appointment and $100.00 for appointments for new patients, physical examinations or thyroid ultrasound.
Payment and Insurance Claims
Dr. Sklar does not participate with any health insurance plans. He is a non-participating Medicare provider. Payment is expected at the time of the appointment. As a courtesy, his staff will file a claim with your insurance company for reimbursement. Our office takes cash, checks and all credit cards for payment.
